Treatment – Adult

The most common types of adult scoliosis include primary de novo degenerative scoliosis and progressive idiopathic scoliosis. Primary degenerative scoliosis represents a new (de novo) curve that develops in patients with no prior history of scoliosis, and typically affects the lumbar or thoracolumbar spine. Progressive idiopathic scoliosis represents curve progression and spinal degeneration in adults with pre-existing idiopathic scoliosis. 

According to research the prevalence of primary degenerative scoliosis has been reported to be as high as 68% in individuals aged 60-90 years. 

While degenerative processes are seen in a vast majority of the population with normal aging, what varies are the mechanical, nutritional and inherited factors that can lead to more rapid progression potentially resulting in significant pathology.

In those patients that develop adult degenerative scoliosis a crucial pathophysiological concept to appreciate is the asymmetry of the disc and facet joint degeneration which leads to progressive imbalance of axial loading and subsequently spinal deformity. 

It is proposed that changes in the structure, function and physiological alignment of the spine lead to asymmetrical loading which provokes further degenerative change, accelerated curve progression and postural collapse.

Patients with adult degenerative scoliosis most commonly present with axial back pain as their primary complaint. The frequency of this is reported to occur in up to 90% of cases.

Adult degenerative scoliosis is a combination of postural disorganization, coronal and sagittal imbalances which then leads to pain. 

The main aim when treating adults with scoliosis is to reduce pain, improve their posture and try to prevent further progression of the curve.
